The supplier can send the license to the NSC with a signed cover letter by the authorized or delegated official. The full 855s is not necessary. Now if they are adding a new state where they will be providing services, they will need to complete the appropriate sections of the 855S including the products/services in section 2 and section 4 requesting the state(s) they intend to serve.

I checked with NSc regarding the need to submit an 855S if I will be serving more than one state (in my case a small part of PA and all of NJ). They informed me that I did not have to submit one if I had not added a location or if the majority of claims submitted are still coming from the same state as listed with the NSC (Section 4). If I added a location or the majority of claims submitted for the listed location changes then I must submit the 855S.

David as long as that state does not require a license you are OK but if a license is required you have to let the NSC know you have the license especially in CB area.

I recently was approved by the NSC for my revalidation. Recently, I discovered that we were getting some denials for our 02 patients (CO-172)
I called the NSC and spoke with the analyst that sent me the approval letter. She said I was missing information in our enrollment packet as a,” Medical Supply Company with Respiratory Therapy”. I asked if she had the HMDR, exemptee license, and she said that my Respiratory Therapist cards were not clear. I faxed the RT cards to her, and she said it was good, and would update my file.
I called the NSC a week later, and used the voice prompts. The recording said that there is request for addtional information. I am so confused. What information do I need to send? The analyst never made it clear of what she wanted, and I thought it was good by the resending of my RT Cards. All the items I sent were current, and not expired. Help, Joe
