The supplier can send the license to the NSC with a signed cover letter by the authorized or delegated official. The full 855s is not necessary. Now if they are adding a new state where they will be providing services, they will need to complete the appropriate sections of the 855S including the products/services in section 2 and section 4 requesting the state(s) they intend to serve.

UPDATING your state Licenses with the NSC
2012 Updates on PWC’s
NEW for 2012 – all PMDS /including scooters except group 3 complex rehab chairs will go on pre-pay review in 7 states –CA/FL/IL/TX/NC/MN/ & NY. Prepay is BAD for the industry — see AAH homecare and industry comments on the US Rehab home page — the pre-pay review will last from 3 to 9 months — this was going to be on ALL but CMS came back and changed to maybe 60% will be reviewed.
The pre-pay will be completed in a staggered series for different states and then prior authorization will be implemented. The full demonstration will be for 3 years. Once the pre-pay has been completed.
Prior Authorization — good for all
Please contact your legislators and let them know this will definately deny access to needed individuals and cause loss of jobs and close down small providers. Pre-pay means a delay of several months for payment on your claims (this will be first month review only on capped rental)

CMNs or Confirmation of Order
CMN forms are only required on 5 items — Oxygen / Tens units (purchase) / Seat lift mechanisms/ Osteo genises Stims & Lymphadema pumps — What suppliers are calling CMNs are really just an order for the doctor to review and sign off on. It is important to realize that this should not be something that you just print off from your computer system. There needs to be something explaining from whom the order came/ their title / date and time. this means that a physicians office/snf/hospital discharge or someone called/faxed or electronically sent you a request for an item for a specific patient. Each jurisdictions has an exact description of a dispensing order and a detailed written order as well as WOPD (written order prior to deliver) {which is required on certain items)
